Dermal Fillers At Tribeca Medaesthetics

A dermal filler is an injectable treatment that can help to reduce the appearance of wrinkles, add volume to your face, and create smoother contours. Patients also use fillers to add fullness to the lips or cheeks. Typically containing either hyaluronic acid or poly-L-lactic acid, these injectables work with your skin to hydrate and plump your complexion. Providers place them in strategic locations to add contour and shape to the facial profile.

At Tribeca Medaesthetics, we offer several types of dermal fillers that we use to add volume, structure, and fullness to your skin. Under the guidance of Dr. Jason Shapiro, an experienced anti-aging physician, we specialize in providing natural-looking results that restore a more youthful look. Whether you want to erase fine lines and wrinkles or add more volume to your lips and cheeks, Dr. Shapiro can achieve the aesthetic goal you desire.

How Do Dermal Fillers Work?

Hyaluronic Acid

To alter and add volume to the skin, dermal fillers may use substances that are already present in the skin. Hyaluronic acid (HA), for example, can hold a tremendous amount of water and is present in the epidermis, dermis, and the deeper layers of the skin. Throughout cellular functioning, HA links with the collagen in the skin to reinforce its strength and thickness. (1) When a patient starts to age, the skin quickly loses moisture and the production of important elements like HA slows down. Dermal fillers work to reintroduce the substance into the skin where it is needed the most.

Two HA dermal filler brands offered at Tribeca Medaesthetics are:

  • Juvederm: Often implemented to add volume and contour to the cheeks and lips, Juvederm has a variety of dermal fillers based on their thickness or viscosity that can target different treatment areas to rejuvenate the face. They all have a very long shelf life and are easily customizable to meet each patient’s needs. (1)
  • Restylane: Restylane is a brand of dermal fillers made from hyaluronic acid used to add volume and fullness to the face, thus reducing the appearance of wrinkles and folds. It is commonly used in non-surgical cosmetic procedures to enhance facial features, such as lips, cheeks, and under-eye areas.

Sculptra (Poly-L-Lactic Acid)

Where HA simulates and produces moisture and strengthens the skin, poly-L-lactic acid (PLLA) stimulates the skin’s collagen production by interacting with fibroblasts key cells in the extracellular matrix, the space between cells in the skin. When introduced under the skin, this substance triggers our natural healing response and stimulates collagen production. Instead of literally filling the depleted areas themselves, the injection process of PLLA fillers slowly builds fibrous tissue and/or collagen that restores facial volume. (2)

At Tribeca Medaesthetics, Dr. Shapiro prefers to use Sculptra for patients looking for an uptick in their collagen production. This dermal filler is FDA-approved to add volume and thicken the dermis by acting as a dermal matrix. Sculptra is biodegradable and biocompatible and is considered to be a “biostimulatory filler.” This is because it replicates wound healing mechanisms like the building of fibrous tissue to “fill” and replace lost volume for the patient. (2)

“Non-surgical procedures like Restylane, Juvederm, and Sculptra are an excellent anti-aging choice for patients interested in looking more youthful without the need for expensive and inconvenient cosmetic surgery.” -Dr. Shapiro

Ideal Candidates

Many factors can contribute to whether you are a good candidate for dermal fillers. Most often, those who use dermal fillers are in their 30s looking to respond to signs of premature aging. People in their 20s might also be interested for this reason. Those who are 40 and up can also benefit from the anti-aging effect of dermal fillers. Most patients qualify for dermal filler treatments as long as they are not allergic to the ingredients inside their chosen injectable. Because there are many different types of injectables, patients have seemingly endless possible combinations at their disposal to achieve the desired result.

During your consultation, Dr. Shapiro will analyze your facial structure and discuss your anti-aging goals to determine which dermal filler is right for you. He will then create a customized treatment plan for you to address your aesthetic needs. A member of our experienced staff will assist you in scheduling your next appointment with Dr. Shapiro.

Dermal Filler Preparation and Procedure

There is little necessary preparation required before dermal filler injections. You should protect your skin from prolonged sun exposure and keep your skincare routine simple before and after dermal filler appointments. You should arrive at your appointment with freshly cleansed skin and may need to account for extra time if Dr. Shapiro feels a numbing agent may be necessary to increase your comfort.

During your treatment, he carefully injects your chosen filler to contour, smooth, and plump your areas of concern. This may involve multiple small and precise injections of Juvederm, Restylane, or Sculptra. Dr. Shapiro may utilize more than one type or line of fillers to achieve an optimal result. As there is virtually no downtime, after your treatment is completed, you’ll be free to return to your regular schedule.

Recovery & Results

After treatment, you should wait 24 hours before strenuous exercise and stay out of the sun as much as possible. Try to avoid touching the treatment area for two or three days unless Dr. Shapiro instructs you to gently massage the skin after your injections. You can expect to see your results almost immediately depending on swelling or bruising. If that occurs, you may need to wait several days after the procedure once the healing is completed to appreciate the final results. You can expect dermal filler injections to last anywhere from six to nine months depending on the area of application and the type of filler. Some areas with greater blood flow, like your lips, may last for a shorter time.

Combining Anti-Aging Treatments

Botox & Dysport

Dr. Shapiro offers many different injectables including two types of neuromodulators: Botox and Dysport. Rather than replenishing moisture and collagen, these injectables temporarily stop the targeted muscles underneath the skin from moving to reduce wrinkles and fine lines. Neuromodulators contain medications produced from botulinum toxin. They prevent muscles of facial expression from contracting resulting in the softening of facial wrinkles. When injected in a very precise and strategic way, the end result is a more youthful appearance without looking overly done or frozen.

The treatment process for Botox and Dysport is similar to that of dermal fillers. Dr. Shapiro injects the neuromodulators where your skin wrinkles while making certain expressions. This type of wrinkle is dynamic, meaning it worsens the more you move your facial features. By injecting neuromodulators, Dr. Shapiro smooths these lines to improve your overall complexion. This leads to more youthful-looking skin that can last up to five months.

Some fillers can produce optimal results for up to one year, whereas others last for around six months. This is completely dependent on the type of filler and the treatment area.
